Anxiety Therapy in the Netherlands: CBT, ACT, and What Actually Works

Anxiety is the most common reason people seek psychological support in the Netherlands — and among expats, it is particularly prevalent. The demands of building a life in a foreign country, navigating unfamiliar systems, and performing your entire professional life in a second language create a specific kind of sustained pressure that, over time, can tip into clinical anxiety.

What Is Anxiety, Really?

Anxiety is not the same as stress. Stress is a response to a specific external demand. When the demand passes, the stress reduces. Anxiety persists beyond the trigger — a state of ongoing alertness that continues even when there is no immediate threat.

Core experiences of anxiety:

  • Persistent worry that is difficult to control
  • Physical symptoms: racing heart, muscle tension, difficulty breathing
  • Difficulty concentrating or making decisions
  • Hypervigilance — constant scanning for potential problems
  • Avoidance of situations that trigger anxiety

Anxiety disorders are among the most treatable psychological conditions.

Why Expats Are Particularly Vulnerable

Unpredictability and loss of control. When systems are opaque and unfamiliar, this sustained uncertainty is a powerful anxiety trigger.

Social evaluation in a second language. Many expats experience heightened social anxiety because they cannot fully express themselves or read subtle cues in their second language.

Identity under pressure. Anxiety often increases when there is a gap between who we feel we are and how we are perceived — common in cross-cultural professional settings.

Bureaucratic overload. The Dutch administrative system is complex. For anxiety-prone people, ongoing management of permits, insurance, and tax filings becomes a persistent source of rumination.

Which Therapy Is Most Effective for Anxiety?

CBT — The Gold Standard

Cognitive-Behavioural Therapy has the strongest evidence base for anxiety treatment. It consistently outperforms medication in long-term outcomes.

CBT works on two levels: cognitive restructuring — examining and testing thoughts that maintain anxiety — and behavioural exposure — gradually approaching situations that anxiety has caused you to avoid. Avoidance maintains anxiety; exposure reduces it.

A standard CBT course for anxiety runs 8–16 sessions.

ACT — Acceptance and Commitment Therapy

Rather than challenging anxious thoughts, ACT focuses on changing your relationship to them. The goal is not to eliminate anxiety but to reduce the extent to which it controls your behaviour.

ACT is particularly effective for generalised anxiety, people who have tried to "think their way out" without lasting success, and anxiety tied to a fundamental values conflict.

EMDR for Trauma-Linked Anxiety

When anxiety is directly connected to a past traumatic experience, EMDR may be the most efficient route. Once the traumatic memory is processed, the anxiety responses it triggers typically reduce significantly.
